Targeted Outreach Portal

Community Rebuilders is partnering with the City of Grand Rapids to address the housing needs of persons who are sleeping on the streets. Members of the Targeted Outreach Team may use this portal to assist persons residing in geographically targeted areas in accessing supported solutions.

We want to inform our valued users that CHC 1.0, originally designed to serve the needs of families experiencing homelessness effectively, is currently being utilized by our local Continuum of Care (CoC) as an access point for the Coordinated Entry System (CES) to provide services to a broader population.

While we understand and support the CoC's urgent need to assist a wide range of individuals, this usage may not fully align with the original intent and platform scope of CHC 1.0 or the specific requirements of HUD's CES.

We acknowledge that this has resulted in some challenges for our cherished clients, and we understand your concerns.

We are actively working on CHC 2.0, a comprehensive platform designed to meet both the needs of our clients and HUD's CES requirements. Our commitment to excellence remains steadfast, and we aim to provide a seamless transition to CHC 2.0, where clients can expect improved services and a user-friendly experience.

We appreciate your patience and understanding during this transition period. Rest assured that our priority is to continue serving our clients with the highest standards of care. We look forward to offering you a better and more efficient platform in the near future.

Thank you for your continued support.

Warm regards, Community Rebuilders
